On what platform are you trying this? Your post includes many tags which are not obviously related to your question, but none for the platform.
Based on your description, you should have two Login items for PayPal - each with the same website. You should be able to create a passkey for each of them.
I can't test this, as today, like every day since PayPal implemented passkey support, I am prevented from creating a passkey in my web browser, with the site advising

where the FAQ mentions a small selection of specific platforms and clients despite passkeys being a standards-based technology not subject to such limitations.
